NHS 111 is a free to call telephone service  in Yorkshire, to call if you need medical help fast, but are not in a life-threatening situation - available 24 hours a day, 365 days a year, for people who are not sure if they need to go to A&E, do not have a GP to call, or generally need reassurance and advice. Just dial 111.

New Patient Registration

stethescopeFrom 2015 the doctors welcome new patients who reside within and outside our contracted area (see Practice Area). 

Patients who live outside our contracted area, would enjoy all the benefits of our services with only one exception - if you are in need of a home visit, you need to contact 111. For more details, please see the information below

Out of Area Registrations - Info for NEW Patients with an Out of Area Address

From 1 September 2016 we are introducing ID checks for prospective patients over the age of 16, the only exception being residents of care homes.  Acceptable forms of ID are: driving licence, utility bill or bank statement from the last 3 months.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered an appointment with the health care assistant. 

Medical treatment is available from the date of registration. 

You may print off the following forms, fill them out, and bring them in with you on your first visit to the practice, or post them to us.
