New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.

From 1 September 2016 we are introducing ID checks for prospective patients over the age of 16, the only exception being residents of care homes.  Acceptable forms of ID are: driving licence, utility bill or bank statement from the last 3 months.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered an appointment with the health care assistant. 

Medical treatment is available from the date of registration. 

You may print off the following forms, fill them out, and bring them in with you on your first visit to the practice, or post them to us.
